Add a qualifying Google Ads lead form to a Search campaign
A drafted Google Ads lead form with one qualifying question, previewed and attached to your campaign only after your approval.
When to use it
When mobile searchers click your Google Ads but drop off on the website, and you want them to leave their details in a quick form instead.
What you get
A previewed lead form with fields, one qualifying question and character counts, attached to your Search campaign only after you approve, plus how to pull submissions.
Does it change anything in my account?
Only with your approval. The assistant first shows a preview of every change and waits for your go-ahead.

In Google Ads account [account name], I want to add a lead form to Search campaign [campaign name] so mobile searchers can leave their details without loading the website.
1. Use list_extension_assets and list_assets to check whether a lead form already exists on this campaign or in the asset library. If one exists, show me its fields and stop. We should not run two forms side by side.
2. If none exists, draft a new form with these elements:
- Headline and description.
- Fields: name, phone and email only.
- One multiple-choice qualifying question: [qualifying question, e.g. monthly budget], with 3 to 4 answer options.
- Call to action and submission message.
- Privacy policy URL: [privacy policy URL].
3. Show the full form as a preview, including the character count for each text field, and state whether it will attach at campaign level.
4. Wait for my approval before creating or attaching anything.
After creation, confirm the form is attached. Then explain how I pull submissions with list_lead_form_submissions so sales can work them daily.
